✔️ Conduct psychosocial assessments & supportive counseling
✔️ Collaborate with providers to create integrated care plans
✔️ Deliver case management & resource coordination
✔️ Maintain accurate EHR documentation
✔️ Participate in team-based care meetings
✅ Qualifications
✔️ MSW from an accredited program
✔️ Active NYS LMSW license (Required)
✔️ 1+ year of experience
✔️ Fluent in Spanish (Required)
✔️ Skilled in assessments, counseling modalities, case management, and EHRs
✔️ Compassionate, culturally competent, and organized
